Social Services
The Social Service Department at Wishek Community Hospital & Clinics provides medically-related social services to patients, patient's families and hospital personnel. Services are provided by a Licensed Social Worker and include psychosocial assessment of the needs of the patient and the patient's family, when appropriate, and arrangement, coordination, monitoring, evaluation, and advocacy for a package of multiple services to meet the specific patient's complex needs.

Outpatient Continuity of Care Information, including:
Physical Therapy
Skilled Swing Bed Program
Home Health Programs
Home Care Options
Area Assistance Programs and
Agencies
Advance Directive Assistance
Referrals to Social Services programs are accepted from physicians, patients, and patient families.
